Dialectical Behavior Therapy (DBT) was developed by Dr. Marsha Linehan in the late 1970’s and throughout the 1980’s. After years of practice with Cognitive Behavioral Therapy (CBT), Dr. Linehan realized that many people struggled with CBT’s therapy system and something new was needed to address their needs. Through years of research and clinical trials, she developed DBT as an effective alternative to CBT with a greater emphasis on skills usage, emotional regulation, and more to help clients experience unprecedented results.

The skills-based system and emotional regulation strategies in DBT were found to be almost universally helpful to clients looking to regain control of their thoughts and emotions. Dialectical Behavior Therapy for Adults follows the classic DBT curriculum combining individual therapy and skills lessons to cultivate both acceptance and growth. DBT for Adults helps clients enhance their present moment awareness, work with challenging thoughts and emotions, implement proactive emotional regulation strategies, and create healthier, long-lasting relationships.
DBT-A adapts DBT for teen clients with different ways of presenting concepts to face the unique challenges of adolescence including navigating school, social pressures, physical development, and growing up. In addition, modern teenagers face a number of unique challenges that older generations never faced such as cyberbullying, social isolation, staying safe in online communities, and much more.
DBT-C primarily focuses on teaching both children and parents how to work together to create a healthy relationship through skills usage and behavioral shaping. Parents are key to DBT-C since children can struggle with skills acquisition and practice inherent to DBT programs. DBT Classic follows the original structure of DBT as developed by Dr. Marsha Linehan with both individual therapy sessions and 2-hour long skills groups each week with optional phone coaching in certain circumstances. DBT Classic is a highly demanding version of DBT, but it has been proven effective by research time and time again. Clients are expected to participate in both individual therapy sessions as well as group skills classes featuring both structured lessons and group sharing and discussion. Unfortunately, DBT Classic is not covered by most insurance companies in Texas and portions of the program require premium pricing.
Developed and written by Dr. Sylvia Gearing, GearingUP’s One-on-One DBT is a modified version of DBT that restructures the program into ongoing one-on-one sessions with a DBT therapist. Clients complete the entire DBT program through a mixture of skills sessions and individual therapy, but they receive the personalized attention of one-on-one sessions with a DBT trained therapist. Many clients prefer the one-on-one format so their DBT sessions are focused solely on them, and they can receive personalized attention in a private, judgment free environment. Since all sessions are completed in an individual therapy format, One-on-One DBT is covered by most insurance policies.
GearingUP’s DBT Telehealth program allows you to complete the full DBT program remotely from the comfort of your home. Even if you have a long commute, travel for work, or live outside the Dallas area, GearingUP’s entire DBT program is available through telehealth sessions. DBT Telehealth is perfect for clients who have unpredictable schedules, travel regularly, or are simply looking to bring the convenience of telehealth to this life changing therapy! Most insurance policies cover DBT Telehealth, but we would be happy to verify your benefits if you contact our office. Note: due to requirements of Texas state law, telehealth services are only available to clients currently in the state of Texas at the time of the appointment.
DBT Standard is a modified version of DBT developed by Dr. Sylvia Gearing to make DBT more accessible for those with busier schedules. Clients meet with an individual therapist and complete only one hour of group DBT Skills class per week with options to reschedule the skills group portion to an individual skills “make up” session for flexibility. Dr. Sylvia also infused the program with the latest research and cutting edge science from many disciplines including neuroscience, other therapies, and more. Due to the modified presentation format, many insurance policies cover DBT Standard but some do not cover portions of the program.
DBT Essentials is a stripped down version that presents only the basics of the DBT curriculum. This format is only offered on a case-by-case basis at the discretion of the senior DBT leadership team, and it is intended only for certain clients in particular circumstances such as: limited time and availability or a particular deadline for completion, clients looking to “refresh” their DBT knowledge after completing a DBT program elsewhere, or more. DBT Essentials must be completed in a one-on-one format with a senior DBT therapist, and it is usually covered by insurance depending on your policy benefits.
DBT Intensive doubles the pace of the DBT program for those looking to complete the full program in a short amount of time. Traditional DBT programs often require 12-24 months to complete, but DBT Intensive can be completed in less than 12 months depending on the client’s circumstances. DBT Intensive is intended for particular types of DBT clients such as those experiencing more severe symptoms or diagnoses, “high risk” populations, or clients who have been discharged from higher levels of care. DBT Intensive is also for clients who are simply motivated to complete DBT in a shorter amount of time. Insurance coverage is determined by individual policy benefits, but our billing office would be happy to verify your insurance benefits on your behalf.
Mindfulness is the foundational skill of DBT, and every skill that follows relies on mindfulness to be effective. Mindfulness teaches us how to pay attention to the present moment on purpose and without judgment. GearingUP teaches dozens of different ways to practice mindfulness to find an effective method that works for you.
Distress Tolerance skills focus on accepting our current circumstances and using skills to help us work through intense emotions in the moment with effective means. GearingUP also teaches clients effective ways to gauge their emotions and the intensity of their experiences for more effective skills usage for any level of distress from mildly upset to crisis situations.
Emotion Regulation skills help us proactively identify and manage our thoughts and emotions, cultivate more positive emotions in our daily lives, and even change our emotions for more beneficial experiences while building a life worth living. Over time, GearingUP clients learn how to have the emotion they want, at the intensity they want it, and at the time that is most beneficial for them.
Interpersonal Effectiveness prepares us for interacting with others out in the world. Many of us can be triggered by the people around us, by our circumstances and experiences, and by the world around us. GearingUP clients learn how to maintain healthy relationships with others both out in the world and at home for a happier, more connected life.
Dialectical Behavior Therapy (DBT) has a number of benefits compared to other therapy systems:
DBT places a central emphasis on understanding our emotions, how thoughts and emotions impact our behavior, and strategies for better emotional regulation and skills usage. Understanding how our emotions shape our decision making, influence our thoughts, and change our behavior is critical for self regulation and building a life worth living. Many clients struggle with other therapy systems because extreme emotions can feel so overwhelming and can overload the “logical mind.” DBT was built from the ground up to address extreme emotions and empower us to regain control.
Dialectics refers to two opposing ideas being true at the same time. For instance, “I am doing the best that I can” and “I can do better” may seem contradictory on the surface but in fact both can be true in the same moment. This engagement with complexity and nuance differs from many other therapy systems where clients may feel like they are being told to “just think differently.” Purely change focused approaches can feel invalidating and cause even more dysregulation than a more compassionate and validating therapy such as DBT.
In study after study, researchers found that the magic of DBT was in its curriculum of over 100 coping skills divided into the four key areas of Mindfulness, Distress Tolerance, Emotion Regulation, and Interpersonal Effectiveness. Skills are taught to clients so they have the tools they need in the moment when adversity strikes whether it’s a momentary flash of anxiety or a full meltdown. Skills allow clients to choose what’s going to be most effective and what’s most comfortable for them, and clients are able to use skills years or even decades after therapy has concluded.
Many therapy clients struggle with “therapy as usual” with a non-directive approach where the client directs the conversation each session with updates, stories, or “the problem of the week.” While there is space for individual discussion and personal updates in DBT therapy, DBT is a structured therapy where each session has a goal to make sure the client is making progress towards their goals and building a life worth living. The structured nature of DBT also allows clients to easily converse with anyone familiar with DBT including the entire DBT treatment team or additional professional resources.
While other therapies may work for more straightforward issues, DBT was developed specifically for clients with more complex situations. Clients may have multiple issues occurring at the same time or feel torn between different ends of a dialectical dilemma, they may experience overwhelming thoughts or emotions that make it difficult to use the interventions of other therapies, or they may have difficulty maintaining therapeutic relationships or relationships in their personal lives. DBT grapples with complex situations and helps clients understand how to effectively manage and cope multiple stressors at once.
In long term studies of clients who have completed DBT treatment, researchers found benefits for years or even decades to come. Due to the skills based curriculum, clients were able to maintain their results over the long term and “self service” when temporary crises may arise. Whereas “therapy as usual” may suggest clients need to return to therapy, the goal of DBT is to empower clients to take care of themselves no matter what comes. Of course, any client can return to DBT treatment in the future if they would like to refresh or work on new goals.

Yes, all of GearingUP’s DBT therapists are either certified in DBT or are eligible for DBT certification in the near future. DBT certification requires clinicians to meet a very high standard of training and experience including dozens of hours of formal training, direct supervision with a DBT expert, a certain amount of direct hours practicing DBT therapy, and the clinician must be fully independently licensed. Certain GearingUP therapists have met all necessary criteria for DBT certification in terms of DBT training and supervision, but they may have to wait for official certification until they have completed all requirements such as finishing a required amount of hours of experience, full licensure, or other requirements.
As of 2024, GearingUP has chosen to not pursue “Linehan Board Certification” because GearingUP could no longer offer DBT on health insurance. Behavioral Tech, Dr. Marsha Linehan’s institute that oversees “Linehan Board Certification,” requires all Linehan Certified DBT therapists to strictly adhere to the “classic version” of DBT as originally developed by Dr. Linehan. Dr. Linehan’s original DBT protocol requires one or multiple individual therapy sessions per week, one or multiple 2+ hour long DBT skills group classes per week, multiple rounds of phone coaching with DBT therapists per week, and more.
As of this writing, insurance companies in the state of Texas will not cover such intensive forms of treatment in an outpatient setting (outside of a hospital). Further, GearingUP would not be able to offer multiple versions of DBT programming to make DBT treatment more flexible and accessible for clients with options like the DBT program presented entirely through individual sessions, shorter DBT skills group classes (around one hour long vs 2+ hours), more or less intensive versions of DBT based on client needs, etc. In an effort to keep DBT accessible, affordable, and realistic for most outpatient clients, GearingUp will not pursue “Linehan Board Certification” unless Behavioral Tech’s policy changes in the future.
The name of Dialectical Behavior Therapy refers to one of the core ideas of the system – Dialectical Thinking. Classic DBT with the intensive schedule is estimated to last around 6-12 months with clients attending every session, every week. However, some clients choose to continue DBT therapy to review previous material, confirm their skills mastery, or for other reasons. Clients may also lengthen or shorten the duration of their DBT treatment depending on which form of DBT they choose or what is recommended for their particular needs.
